Mount Airy High School has implemented a new process for Student-Athlete Physical Packets. Please login to the DragonFly account that you created last year to complete the required forms.  If you do not have an account, you can use the following link to create one:

 

  

Please review the DragonFly "Getting Started" file below prior to beginning. You still need to print the Physical Form located at the bottom of this page and have it completed by a physician to show proof of a medical exam. This can be uploaded directly to your DragonFly account.  If your student-athlete received a free physical at MAMS or MAHS, we have those forms and they will be uploaded for you once you create an account.
